Going home - what's next?
During or after your stay at the Netherlands Cancer Institute you may need extra help. It can be beneficial to consider your needs early on so you can make the necessary preparations.
What can you do?
Before your hospitalization or admission, spend some time thinking about the kinds of things the people in your network (family, friends, neighbors) can offer. Can they help you around the house, preparing food, or getting groceries? Consider consulting a grocery service, meal service, or similar services as well. And make sure to arrange transport to take you home after your stay.
If you think that you may need help around the house, please contact the WMO in your municipality.
The transfer nurse
You may not be able to (fully) count on your personal network for all the help or support that you need, despite being dependent on it. If necessary, a transfer nurse can help you find the care that you need after your discharge from the hospital. Within the legal frameworks, a transfer nurse can organize the essential professional care for you, in collaboration with you and your loved ones as well as other disciplines involved.
Patients who have not been admitted to the hospital can come to the Patient Information Center to learn more about the arranging of care, and where to find it.
